ICMP Application Checklist

ICMP Fujitsu Scholarship applicants must submit completed documents in the Application Submittal Requirements section listed below.

Deadline for submission: November 30, 2006
All application documents, test scores and academic transcripts must be received by JAIMS postmarked by November 30, 2006. Applicants who fail to submit all documents by the due date cannot be considered for the Fujitsu Scholarship.

  • PDF Two Recommendation Forms (57KB / A4)
    Note: Print two copies of the recommendation form - two recommendation letters are required.
  • ICMP Application Form (411KB / A4)

    Note: The Health Form should be submitted after notification of admission to ICMP.
  • TOEFL, TOEIC or IELTS score report. Valid test dates are between November 2004 and November 2006.
  • Official academic transcripts.
    Academic transcripts must be sent directly from each institution to JAIMS.
